Steps for Registration

All forms on this page must be completed by all schools attending the provincial tournament. This includes every form in Step 1.
All forms are found at the bottom of this page.

Step 1:

Download and complete the below waiver forms. These forms must be completed and submitted to your school principal.
Form 1: Student
Form 2: Coach/Volunteer

For your school principal: Download and complete the below waiver forms. These forms must be completed by the principal and submitted to (ASAA Office - info@asaa.ca)
Form 3: Principal (for Students)
Form 4: Principal (for Coach/Volunteer)

Step 2:
