Steps for Registration

All forms on this page must be completed by all schools attending the provincial tournament. This includes every form in each of the below steps.

Step 1:

Download and complete the below waiver forms. These forms must be completed and submitted to your school principal.
Form 1: Student
Form 2: Coach/Volunteer

Schedule “A” to the ASAA Acknowledgement and Waiver Form - Students/Parents/Guardians
Schedule “A” to the ASAA Acknowledgement and Waiver Form - Coaches and Event Volunteers

For your school principal: Download and complete the below waiver forms. These forms must be completed by the principal and submitted to (ASAA Office - fill in ASAA contact)
Form 3: Principal (for Students)
Form 4: Principal (for Coach/Volunteer)

Step 2:

Download and complete the ASAA Provincial Roster Form and save it as an Excel file.

Step 3:

Fill out the Provincial Championship Registration Form.
*You will need to upload your completed ASAA Provincial Roster Form, Team Photo and Team Logo to the Registration Form.

Step 4:

Prepare event payment (see instructions below)
The deadline to register is Monday, March 14, 2022 at 12:00 P.M.
